This site provides online coaching to caregivers of individuals diagnosed with Alzheimer's disease or another dementia, who reside within the service areas defined below. A health care professional serves as your coach and provides personalized education, support, guidance, referrals and care planning assistance.

Eligibility Requirements

●	Either the caregiver or the person diagnosed with dementia must reside within the following service areas (see About Us page for county listings):
		o	Greater Cincinnati Chapter – 27 counties in southern Ohio, northern Kentucky and southeastern Indiana.
		o	Cleveland Area Chapter – 5 counties in northeastern Ohio
		o	Northwest Ohio Chapter - 24 counties in northwestern Ohio
●	The caregiver must have regular internet access and an active email account.
●	The caregiver must provide requested demographic information about the caregiver and the person diagnosed with Alzheimer's/dementia.
●	The caregiver must participate in evaluation of this program.
